El papel de Enfermería en la práctica clínica transfusional desde una perspectiva de seguridad

Abstract

Introducción. La seguridad de los pacientes en el campo de la medicina transfusional en los últimos años ha adquirido gran relevancia. Se estima que alrededor de un 3% de los pacientes transfundidos experimentan algún tipo de efecto adverso o reacción transfusional. Se ha identificado que la causa más frecuente de este problema es el error humano en la cabecera del paciente cuando no se comprueba que se está administrando la sangre correcta al paciente adecuado, siendo el principal responsable de esta actuación el personal de enfermería. Objetivos. Con este trabajo se pretende identificar la relevancia del papel de enfermería en la transfusión sanguínea desde una perspectiva de seguridad. Métodos. Se ha llevado a cabo una búsqueda bibliográfica en diferentes bases de datos, (PubMed, Scielo, Cuiden, Dialnet, Scopus y Google Académico), así como en revistas, libros, guías y manuales para conocer lo que se ha publicado entre los años 2006 y 2016 sobre la terapia transfusional y el papel de la enfermería en ella. Resultados/Conclusiones. El papel de enfermería en la práctica transfusional es clave para garantizar la seguridad en la transfusión sanguínea. La literatura revisada evidencia escasez de conocimientos y habilidades técnicas como una de las principales causas del error enfermero. Se considera oportuno seguir trabajando en la línea de aumentar la formación y el entrenamiento de enfermería, establecer más medidas de apoyos que sustenten su trabajo y la plena inclusión en los sistemas de hemovigilancia españoles.

Introduction. In the field of the transfusion medicine, safety in patients has acquired great relevance over the past years. It is estimated that approximately 3% of all transfused patients experiment some sort of side effects, transfusion response or reaction. It has been identified that the most frequent cause on this problem is human error due to the lack of ensuring that the blood, which is being administrated to the patient, is the right one; therefore the main responsible of this action would be the nursing staff. Objectives. The purpose of this study is to identify the importance of the nursing staff in blood transfusions from a safety´s point of view. Methods. A bibliographical research based on many different databases (PubMed, Scielo, Cuiden, Dialnet, Scopus y Google Academic) was carried out. Magazines, books, guides and manuals were part of the research as well, in order to find out what was being published in terms of transfusion therapy and the nursing role. Results/ conclusion. The role of nursing in transfusion practice is key to ensuring safety in blood transfusion. Literature review shows a shortage of knowledge and technical skills as one of the main causes of nurses’ error. To continue working on the line to increase training and nursing training is considered appropriate, establishing support measures to help its work and full inclusion in Spanish haemovigilance systems.
