Enfermedades raras desde otra perspectiva : la gestión de casos en enfermería

Abstract

Actualmente, se están sucediendo cambios sociales, demográficos y de estilos de vida, que unido al aumento de enfermedades crónicas está haciendo que surja la necesidad de cambio en los sistemas sanitarios, hacia una forma de gestión más sostenible, capaz de hacer frente a las nuevas necesidades de las enfermedades crónicas. En este sentido, surgen los modelos de gestión de casos, dirigidos precisamente a aquellos pacientes crónicos con más necesidades, y, por tanto, más complejos. Unos de estos pacientes son aquellos que tienen una enfermedad rara. Debido a sus características de baja prevalencia y curso crónico, las enfermedades raras requieren un abordaje coordinado y multidisciplinar. Este abordaje es exactamente lo que persiguen los modelos de gestión de casos, en los cuales se incluye al paciente, su entorno y a todos los profesionales y ámbitos implicados para poder prestar una atención integral y coordinada, tan necesaria en enfermos crónicos, y, por ende, en afectados por enfermedades raras. Lo que se persigue, por tanto, con este trabajo, es enmarcar el papel de la Enfermera Gestora de Casos, como líder de los modelos de gestión de casos, y relacionarla con las enfermedades raras y la utilidad que puede tener en este tipo de enfermedades.

Actually, there are happening social, demographic and lifestyle changes that, joined to the increase of chronic diseases, are provoking the arising of the need of change in the sanitary systems in search of a form of more sustainable management; capable of facing the new needs of chronic diseases. In this regard, there arise the models of case management, directed precisely to those chronic patients with more needs, and, therefore, more complex. Some of these patients are those that have a rare disease. Due to his characteristics of low prevalence and chronic status, the rare diseases need a coordinated and multidisciplinary approach. And that approach is exactly what there chase the models of case management intend. They include the patient, his environment and all the professionals and areas involved for being able to give an integral and coordinated attention. That is really necessary when dealing with chronic patients, and, as a consequence, with people affected by rare diseases. What is pursued, therefore, by this work, is to frame the role of the Nurse Case Management, as leader of the models of case management, and to relate the usefulness that this type of nurses can have in the treatment of rare diseases.
