Antecedentes familiares y cánceres colorrectal y de mama

Abstract

INTRODUCCIÓN: Los antecedentes familiares (AF) de cáncer de colon (CCR) y de mama (CM) están asociados con un riesgo incrementado de cáncer en estas localizaciones. El objetivo de este estudio es analizar esta asociación y los factores que la modifican, así como la influencia de AF de otras localizaciones tumorales. METODOS: Se ha realizado un estudio de casos y controles, seleccionando casos incidentes de CCR o CM del área de influencia del hospital Valdecilla y controles elegidos al azar a partir de las listas de población asignada a dos centros de salud, emparejados por sexo y edad a los casos. Como medida de asociación se calcularon Odds Ratio (OR) ajustadas por potenciales confusores mediante regresión logística. RESULTADOS: Globalmente los AF de cáncer duplicaron el riesgo de CCR (OR=2.08), presentando la mayor asociación los AF de CCR (OR=4.31) y digestivo (OR=2.59). No hubo asociación con otras localizaciones tumorales. Respecto al CM: el AF más fuertemente asociado fue el de CM (OR=5.28), aumentando dicha asociación en premenopáusicas y menores de 65 años. Globalmente, los AF de cáncer triplicaron el riesgo (OR=2.87). No encontramos asociación con AF en otras localizaciones. CONCLUSIONES: Los AF de cáncer son un claro factor de riesgo para el desarrollo de CCR y CM, variando su influencia de forma importante en distintos subgrupos. ABSTRACT: BACKGROUND: Family history of colorectal and breast cancer has been associated with an increased personal risk of these cancers. The main objective is determining the association degree and the modifying factors, as well as the influence of family history of other cancers. METHODS: We analyzed the association between family history and cancer risk by using a population-based case-control study. Cases were incident cases of colorectal and breast cancer who lived in the M. de Valdecilla Hospital area. Controls were randomly selected from lists of patients from Primary Care, matched with cases by age and sex. Odds Ratios (OR) were adjusted by the main confounding factors. RESULTS: Concerning colorectal cancer, cancer family history is associated with a doubling of risk for colorectal cancer (OR=2.08) and this risk is greater for colorectal cancer family history (OR=4.31) and for family history of digestive tumours (OR=2.59). There is no relationship with family history of cancer in other locations. Regarding breast cancer, cancer family history almost increases the risk threefold (OR=2.87) and this risk is greater for breast cancer family history (OR=5.28), being stronger for premenopausal women and women aged below 65. There is no relationship with family history of cancer in other locations. CONCLUSIONS: These results confirm that cancer family history is an important risk factor for colorectal and breast cancer development, with substantial variations between subgroups
