Tratamiento del cáncer de próstata por estadios

Abstract

El cáncer de próstata es uno de los más frecuentes en la actualidad; según el estadio del mismo, los tratamientos, o la combinación de los mismos, varía. Dentro de las posibilidades está el tratamiento conservador: vigilancia activa y watchful waiting; los tratamientos locales (útiles únicamente cuando la enfermedad no está diseminada): cirugía y radioterapia; y el tratamiento sistémico (en caso de enfermedad localmente avanzada o metastática): hormonoterapia principalmente. Son muchas las discrepancias existentes en cuanto al tratamiento en este cáncer. La falta de consenso se fundamenta tanto en pocos estudios con clara evidencia como en muchos otros que demuestran como varias opciones de tratamiento son igualmente válidas en una misma situación. Esta revisión pretende homogeneizar, en la medida de lo posible, las recomendaciones de las principales guías (EUA, AUA, NICE, AECC), comentando el tratamiento más ampliamente aceptado en cada caso.

Prostate cancer is one of the most frequent cancers at the moment. According to the stage, the options of treatment (or combinations of treatments) vary. Among these options we have conservative treatment: active surveillance and watchful waiting; local treatment (only useful for non-metastatic disease): surgery and radiotherapy and systemic treatment (used in case of locally advanced or metastatic disease): hormonotherapy as the most used one. There is no consensus as long as treatment in prostate cancer is concerned; mainly due to the lack of statistically significant results in most studies done, but also to the fact there are many trials showing more than one effective treatment in a particular situation. This revision has the purpose of homogenize the recommendations of principle guidelines (EUA, AUA, NICE, AECC) focusing on the most accepted treatment in each stage.
