A pesar de los avances en el tratamiento de las Enfermedades Pulmonares
Intersticiales (EPI), particularmente con el uso de antifibróticos como Nintedanib
y Pirfenidona, estos medicamentos presentan perfiles de efectos adversos que
pueden limitar su tolerabilidad y, por ende, su adherencia a largo plazo. Si bien
los ensayos clínicos han caracterizado algunos de estos efectos, la variabilidad
de las respuestas adversas en poblaciones clínicas reales no ha sido recogida
hasta la fecha.
El estudio pretende describir los efectos adversos relacionados con el
tratamiento antifibrótico en pacientes con EPI en una consulta especializada y
analizar posibles asociaciones con variables clínicas y demográficas que puedan
influir en la incidencia y severidad de los efectos adversos. Se incluyó a 73
pacientes con EPI: 16 tratados con Pirfenidona y 57 tratados con Nintedanib. Se
recopilaron datos al inicio del tratamiento y a los 6 meses. La mitad presentó
efectos adversos, mayoritariamente gastrointestinales, requiriendo
modificaciones en el tratamiento. Se observó en ellos mayor frecuencia de
hipertensión, dislipemia e hiperuricemia, además de un deterioro de la función
respiratoria. El estudio aporta evidencia real sobre el perfil funcional, terapéutico
y de seguridad de los antifibróticos, reforzando su eficacia en una ralentización
de la enfermedad.
Despite advances in the treatment of Interstitial Lung Diseases (ILD), particularly
with the use of antifibrotic agents such as Nintedanib and Pirfenidone, these
drugs have adverse effect profiles that may limit their tolerability and,
consequently, long-term adherence. Although clinical trials have characterized
some of these effects, the variability of adverse responses in real-life clinical
populations has not been documented to date.
The study aims to describe adverse effects related to antifibrotic treatment in
patients with ILD in a specialized clinic and to analyze possible associations with
clinical and demographic variables that may influence the incidence and severity
of these effects. A total of 73 ILD patients were included: 16 treated with
Pirfenidone and 57 with Nintedanib. Clinical data were collected at the start of
treatment and at six months. Half of the patients experienced adverse effects,
mostly gastrointestinal, which required treatment modifications. These patients
showed a higher frequency of hypertension, dyslipidemia, and hyperuricemia,
along with a decline in respiratory function. The study provides real-world
evidence on the functional, therapeutic, and safety profile of antifibrotics,
supporting their effectiveness in slowing disease progression.
