Análisis del ahorro farmacéutico derivado del tratamiento de pacientes oncológicos en ensayos clínicos

Abstract

El objetivo de este estudio fue doble: evaluar el ahorro farmacéutico anual asociado al tratamiento de pacientes oncológicos en el Hospital Universitario Marqués de Valdecilla durante los ensayos clínicos realizados a lo largo de 2020, y estimar el coste de terapias antineoplásicas innovadoras no aprobadas por la Agencia Española Medicamentos y Productos Sanitarios que los pacientes reciben como tratamiento experimental en un ensayo clínico. Se ha optado por un abordaje descriptivo de naturaleza económica, analizando los costes de las terapias con indicación aprobada por la AEMPS para la patología de cada ensayo clínico analizado y cuantificando el valor de la inversión derivada de las terapias innovadoras sin indicación aprobada por la AEMPS utilizadas como tratamiento experimental de un ensayo clínico. Se incluyeron en el análisis 38 ensayos clínicos que contaban con un total de 101 pacientes. La cifra final de ahorro económico en medicamentos fue de 603.350,21€ y una media de ahorro por paciente de 6.630,22€. Por otro lado, el dato final de inversión fue de 789.892,67€, con una inversión por paciente de 15.488,09€. Los resultados obtenidos muestran que los ensayos clínicos, además de ser esenciales para el avance de la ciencia, pueden ser una fuente de ingresos nada despreciable para los sistemas sanitarios, y, por ende, una herramienta que contribuya a su sostenibilidad.

The aim of this study was twofold: to assess the annual pharmaceutical savings associated with the treatment of cancer patients at the Marqués de Valdecilla University Hospital during the clinical trials conducted throughout 2020, and to estimate the cost of innovative antineoplastic therapies unapproved by the Spanish Agency for Medicines and Health Products that patients receive as experimental treatment in a clinical trial. An observational and financial analysis of the drug cost savings was applied. Each clinical trial and the characteristics of the pathology were analyzed and matched with a therapeutic alternative. Direct cost savings to the Regional Health System of Cantabria were measured, related to clinical trials. The cost of innovative therapies used as an experimental treatment in clinical trials was also quantified as an investment. This study includes 38 clinical trials with a sample of 101 patients. The findings indicate that overall, all the clinical trials analyzed provide a total cost savings of €603,350.21 and an average cost saving of €6,630.22 per patient. Furthermore, the final investment amounts to €789,892.67, with an average investment of €15,488.09 per patient. This study demonstrates that clinical trials are essential for the advancement of science. Furthermore, clinical trials can be a significant source of income for both hospitals and Regional Health Systems, contributing to their financial sustainability.
