La cultura de la muerte en el entorno hospitalario

Abstract

La muerte implica una dimensión social, cultural y simbólica que varía entre distintos grupos humanos y a lo largo de la historia. La literatura analizada en este trabajo de revisión, señala que la sustitución del ámbito familiar por el hospitalario como lugar donde se produce el deceso, no siempre se ha preocupado por acoger los aspectos simbólicos y culturales de la muerte y el duelo. A finales del siglo XX y a lo largo del XXI, la medicina ha ignorado que la muerte es una parte básica de la relación médico-paciente y la dinámica hospitalaria, basada cada vez más en la productividad y la eficiencia, ha contribuido a potenciar este desinterés. Este trabajo describe deficiencias formativas y prácticas en cuanto al manejo de la muerte por parte de los profesionales médicos, así como déficits estructurales del propio entorno hospitalario tanto en su dimensión material como organizativa. El análisis realizado, sugiere posibles vías de mejora, que pueden afectar positivamente al paciente, a su familia y a los médicos implicados.

Death carries within it a social, cultural and symbolic dimension that has varied amongst different human groups and throughout history. The literature analysed in this review indicates that, in the replacement of the family setting for the hospital one as the place where death occurs, there has not always been concern to embrace the symbolic and cultural aspects of death and bereavement. At the end of the 20th century and throughout the 21st century, medicine has ignored the fact that death is a fundamental part of the doctor patient relationship, and hospital dynamics, increasingly based on productivity and efficiency, have contributed to this lack of interest. This article describes training and practical deficiencies in the way death is dealt with by medical professionals, as well as structural deficiencies in the hospital environment itself, in both its material and organisational dimensions. The analysis made, offers different options for improvement that might have a positive impact on the patients, families and doctors involved.
