Biología funcional de la glándula tiroides. Hiper e hipotiroidismo

Abstract

La glándula tiroides y la producción de las hormonas tiroideas tienen un papel fundamental en el control del metabolismo. El trabajo consiste en una revisión bibliográfica acerca de la glándula tiroides y su patología más frecuente. En primer lugar, nos hemos centrado en la descripción de los aspectos básicos de su anatomía y exploración. A continuación, hemos revisado la histofisiología de la glándula para comprender su funcionamiento, el proceso de síntesis de las hormonas tiroideas y su regulación, así como las propiedades que estas tienen dentro de la propia glándula y en distintos tejidos. Los siguientes apartados se han centrado en las dos entidades patológicas más representativas y con mayor impacto de la glándula tiroides: el hipertiroidismo y el hipotiroidismo. En cada uno de ellos, hemos abordado la epidemiología, etiopatogenia y las manifestaciones clínicas más representativas junto a complicaciones que pueden desarrollar los pacientes con estas condiciones. Hemos desarrollado el proceso de diagnóstico mediante la analítica y la orientación etiológica, y las distintas modalidades de tratamiento disponibles, tanto médicas como quirúrgicas, sus indicaciones y reacciones adversas. Finalmente, hemos abordado algunas situaciones especiales como el embarazo, la tormenta tiroidea y el coma mixedematoso.

The thyroid gland and the production of thyroid hormones play a fundamental role in controlling metabolism. The project consists of a bibliographic review about the thyroid gland and its most common pathology. First, we focused on the basics of its anatomy and examination. Next, we reviewed the histophysiology of the gland to understand its functioning, the synthesis process of thyroid hormones and their regulation, as well as the properties they have within the gland itself and in different tissues. The following sections focused on the most representative entities with the greatest impact on the thyroid gland: hyperthyroidism and hypothyroidism. In each of them, we have addressed the epidemiology, etiopathogenesis and the most representative clinical manifestations along with complications that patients with these conditions can develop. We described the diagnostic process through analytical data and etiological guidance, as well as the different treatment modalities available, both medical and surgical and their indications and adverse reactions. Finally, we adressed some special situations such as pregnancy, thyroid storm and myxedema coma.
