Hablemos de lesiones por presión: ¿Qué efectividad tienen las medidas preventivas del protocolo del hospital?

Abstract

RESUMEN: Desde tiempos ancestrales las lesiones por presión (LPP) han estado presentes en la humanidad, ya que los agentes que las causan están relacionados con la interacción que el ser humano mantiene con su entorno. Con este proyecto de investigación se pretende conocer si las medidas preventivas recogidas en la “Guía de Actuación en la Prevención y Tratamiento del Paciente con Úlceras por Presión del Hospital Universitario Nuestra Señora de La Candelaria” cumplen con los estándares establecidos en las guías de práctica clínica publicadas hasta el momento para la prevención de LPP. Objetivos: Principal: Analizar si las medidas preventivas incluidas en la “Guía de Actuación de Prevención y Tratamiento del Paciente con UPP” son efectivas para prevenir la aparición de LPP en personas mayores de 65 años. Secundarios: Determinar la incidencia de LPP en las personas incluidas en el estudio, en función de las medidas preventivas a las que están expuestas y evaluar si el nivel de conocimientos de los diferentes profesionales de enfermería es el adecuado para trabajar en la prevención de las LPP. Metodología: Estudio cuantitativo, observacional, descriptivo, longitudinal y prospectivo. La población estará compuesta por los pacientes ingresados en las unidades de geriatría del Hospital Universitario Nuestra Señora de la Candelaria (HUNSC), medicina interna del HUNSC, hospital del Sur y hospital de Ofra. La selección de la muestra se realizará mediante muestreo no probabilístico de conveniencia. Los hospitales seleccionados serán los que componen el Complejo Hospitalario Universitario Nuestra Señora de la Candelaria (CHUNSC): HUNSC, Hospital del Sur y Hospital de Ofra. Se incluirán todos los pacientes mayores de 65 años, con muy alto riesgo de desarrollar LPP (con una puntuación en la escala Norton Modificada de 5 a 11) que ingresen o estén ingresados desde el 1 de septiembre de 2023 hasta el 31 de agosto de 2024, hasta completar el tamaño muestral. Para el cálculo del tamaño muestral se aplicará una técnica de estimación poblacional (proporciones), en la que no habrá una población de referencia ya que se trata de una población infinita. Para ello, se utilizará el software público GRANMO (versión 7.12 para Windows), en el que se volcará un intervalo de confianza del 95%, un valor de α=0,05, una proporción esperada del 5%, un valor de p=0,05, una amplitud o precisión del 3% y un 15% como porcentaje de proporción esperada de pérdidas. Se utilizarán métodos de análisis multivariante y se considerarán significativos valores de p inferiores a 0,05. Los datos serán analizados mediante el programa informático SPSS 24 para Windows. Plan de trabajo: La investigación se llevará a cabo desde noviembre de 2022 y se presentará al Comité de Ética del CHUNSC en febrero de 2023. A partir de marzo de 2023 se esperará respuesta del Comité de Ética, se solucionarán problemas identificados, se formará y adiestrará del equipo de investigación. Posteriormente, se iniciará el reclutamiento de los pacientes y la recogida de datos para, aproximadamente, en septiembre de 2024 realizar el análisis de los datos y confeccionar el informe final durante los meses de noviembre y diciembre del mismo año. A principios de 2025 se realizará la divulgación de la investigación realizada, comenzando por la gerencia del CHUNSC, en el congreso del GNEAUPP y finalmente se publicará en la revista ROL de Enfermería.

ABSTRACT: Since ancient times, pressure injuries (PBI) have been present in humanity, since the agents that cause them are related to the interaction that human beings maintain with their environment. The aim of this research project is to find out whether the preventive measures included in the "Action Guide for the Prevention and Treatment of Patients with Pressure Ulcers (PU) at the Nuestra Señora de La Candelaria University Hospital" comply with the standards established in the clinical practice guides published to date for the prevention of PPI. Objectives Primary: To analyse whether the preventive measures included in the "Action Guide for the Prevention and Treatment of Patients with PUs" are effective in preventing the appearance of PUs in people over 65 years of age. Secondary: To determine the incidence of PUs in the people included in the study, according to the preventive measures to which they are exposed and to review whether the level of knowledge of the different nursing professionals is adequate to work on the prevention of PUs. Methodology Quantitative, observational, descriptive, longitudinal and prospective study. The population will be made up of patients admitted to the geriatric units of the Nuestra Señora de la Candelaria University Hospital (HUNSC), internal medicine of the HUNSC, Hospital del Sur and Ofra’s Hospital. The sample will be selected by non-probabilistic convenience sampling. The selected hospitals will be those comprising the Complejo Hospitalario Universitario Nuestra Señora de la Candelaria (CHUNSC): HUNSC, Hospital del Sur and Ofra’s Hospital. All patients over 65 years of age, with a very high risk of developing LPP (with a score on the Modified Norton scale of 5 to 11) admitted or currently admitted from 1 September 2023 to 31 August 2024, until the sample size is completed will be included. For the calculation of the sample size, a population estimation technique (proportions) will be applied, where there will be no reference population as it is an infinite population. For this purpose, the public software GRANMO (version 7.12 for Windows) will be used, in which a confidence interval of 95%, a value of α=0.05, an expected proportion of 5%, a value of p=0.05, an amplitude or precision of 3% and 15% as a percentage of expected proportion of losses will be entered. Multivariate analysis methods will be used and p values of less than 0.05 will be considered significant. Data will be analysed using SPSS 24 for Windows. Work plan The research will be conducted from November 2022 and presented to the CHUNSC Ethics Committee in February 2023. From March 2023, the response of the Ethics Committee will be awaited, problems identified will be solved, and the research team will be formed and trained. Subsequently, patient recruitment and data collection will begin, with data analysis to be carried out in September 2024 and the final report to be prepared in November and December of the same year. At the beginning of 2025, the research will be disseminated, starting with the CHUNSC management, at the GNEAUPP congress and finally published in the journal ROL de Enfermería.
