Tratamientos tópicos para el control del dolor previo al desbridamiento de heridas crónicas. Protocolo de revisión sistemática

Abstract

RESUMEN: Justificación: El dolor asociado a las heridas crónicas continúa siendo un desafío por la alta implicación y la grave repercusión que tiene para los pacientes, los profesionales y el sistema sanitario. Las consecuencias para el paciente, tanto físicas como psíquicas, deterioran su calidad de vida, pudiendo generar una incapacitación o un problema social. Por tanto, la urgencia en el control del dolor, se vuelve imprescindible para un buen abordaje de la lesión y aún más, cuando utilizamos técnicas invasivas que aumentan la sensación dolorosa. Ante la heterogeneidad para el control del dolor, en la práctica del desbridamiento cortante, surge la necesidad y conveniencia de realizar una revisión de todos los métodos farmacológicos aplicados como anestésicos tópicos. De ahí la importancia de conseguir la mayor información de los métodos, para trabajar con la última actualización posible, siguiendo la evidencia científica. Objetivo: El objetivo principal del estudio es identificar las distintas opciones terapéuticas farmacológicas aplicadas de forma tópica para el manejo del dolor en el desbridamiento cortante. Metodología: Realizaremos una revisión sistemática con un tipo de propósito descriptivo de tipo intervencionista. Iniciaremos la investigación elaborando una pregunta PICO, a partir de la cual realizaremos una búsqueda bibliográfica de artículos durante 9 semanas, en las distintas bases de datos, con sus respectivos tesauros y operadores boleanos. Las bases de datos revisadas serían MEDLINE, CUIDEN PLUS, LILACS, CINAHL, SCOPUS, PRÓSPERO Y COCHRANE. La selección de artículos será realizada por dos investigadores para disminuir el sesgo del investigador principal, participando un tercer investigador si existe discrepancia, otorgando arbitrariedad. Para evaluar la calidad metodológica de los artículos seleccionados se utilizarán las herramientas CASPe. El riesgo de sesgo será analizado por las herramientas Cochrane y para la mejora de la publicación, se utilizará la herramienta PRISMA. Se extraerán y analizarán los datos, contemplando el sesgo de publicación, evaluando la heterogeneidad de los resultados e interpretándolos con el método estadístico de Review Manager (RevMan 5.2.1®). Plan de Trabajo: La realización de la RS se llevará a cabo siguiendo un orden cronológico que muestra las fases que han dirigido nuestro estudio y la duración temporal del trabajo completo. El estudio comienza con una primera Fase Introductoria, seguida de una segunda Fase Metodológica, completándose con una última Fase de Análisis. El periodo de tiempo en el que se realizará y desarrollará todo nuestro estudio, será de 8 meses.

ABSTRACT: Backgound: Pain associated with chronic wounds continues to be a challenge due to the high involvement and serious repercussions for patients, professionals and the healthcare system. The consequences for the patient, both physical and psychological, deteriorate their quality of life and can lead to incapacitation or social problems. Therefore, the urgency in pain control becomes essential for a good approach to the injury and even more so when invasive techniques are used that increase the sensation of pain. Given the heterogeneity of pain control in the practice of sharp debridement, it is necessary and convenient to review all the pharmacological methods applied as topical anesthetics. Hence the importance of getting as much information on the methods, to work with the latest possible update in pain control, following the scientific evidence. Objective: The main objective of the study is to identify the different pharmacological therapeutic options applied topically for pain management in sharp debridement. Methods: We will perform a systematic review with an interventionist type of descriptive purpose. We will begin the investigation by developing a PICO question, from which we will perform a bibliographic search of articles during 9 weeks, in different databases, with their respective thesauri and Boolean operators. The databases reviewed will be MEDLINE, CUIDEN PLUS, LILACS, CINAHL, SCOPUS, PROSPERO and COCHRANE. The selection of articles will be carried out by two investigators to reduce the bias of the principal investigator, with a third investigator participating if there is discrepancy, granting arbitrariness. The CASPe tools will be used to evaluate the methodological quality of the selected articles. Bias will be analyzed by the Cochrane tool and the PRISMA tool will be used to improve the publication. The data will be extracted and analyzed, considering the publication bias, evaluating the heterogeneity of the results and interpreting them with the Review Manager statistical method (RevMan 5.2.1®). Work Plan: The systematic review will be carried out following a chronological order that shows the phases that have directed our study and the temporal duration of the complete work. The study begins with a first Introductory Phase, followed by a second Methodological Phase, and completed with a last Analysis Phase. The period of time in which our entire study will be carried out and developed will be 8 months.
