Utilización de terapias de presión negativa en pacientes con úlceras por presión y el rol de enfermería durante el proceso

Abstract

La Terapia de Presión Negativa es una herramienta utilizada para promover el cierre de heridas, desde quirúrgicas a crónicas, como las úlceras por presión. Este tratamiento consiste en la aplicación de presión subatmosférica mediante un sistema que genera vacío acelerando el proceso de curación de las mismas. Actualmente existen numerosos tipos de dispositivos que no sólo se aplican en el hospital, sino también en el domicilio del paciente, aminorando los costes que se derivan de esta terapia. Objetivos: Definir las características de las úlceras por presión, su etiología y clasificación. Definir la terapia de presión negativa. Establecer un plan de cuidados estandarizado para el paciente en tratamiento con terapia de presión negativa. Para cumplir estos objetivos se ha realizado una revisión bibliográfica, y establecido un plan de cuidados estandarizado para un paciente en tratamiento con terapia de presión negativa, siguiendo los pasos del método científico y apoyándonos en la taxonomía NANDA I, así como en la NOC y NIC para delimitar criterios de resultados e intervenciones enfermeras, respectivamente. Varios ensayos clínicos alcanzan diferencias significativas en favor de este tratamiento. Sin embargo, los estudios son, en general, de calidad limitada debido al escaso tamaño muestral y por ello las complicaciones y efectos adversos no son comparables a los del tratamiento convencional. No existen suficientes pruebas basadas en evidencias concluyentes en el tratamiento de heridas crónicas con este método y, por tanto, su uso generalizado no está justificado.

The Negative Pressure Therapy is a tool used to promote the closure of wounds, from surgical to chronic ones like pressure ulcers. The treatment consists of the application of subatmospheric pressure through a closed system generating a vacuum-like effect, and accelerating the healing process. Nowadaysthere are numerous types of devices that are not only applied in the hospital, but also in the patient's home, reducing the costs derived from this treatment. Objectives: To define the characteristics of pressure ulcers, their etiology and classification. To define negative pressure therapy. To establish a standardized care plan for the patient undergoing treatment with negative pressure therapy. To achieve these goals, a bibliographic review has been carried out, and a standardized care plan has been established for a patient undergoing such treatment with negative pressure therapy, following the steps of the scientific method and relying on the NANDA I taxonomy, as well as on the NOC and NIC to delimit outcome criteria and nursing interventions, respectively. Several clinical trials have reached significant differences in favor of this treatment. However, the studies are, in general, of limited quality due to the small sample size and, therefore, the complications and adverse effects are not comparable to those of conventional treatment. There is insufficient conclusive evidence-based evidence in the treatment of chronic wounds with this method and therefore its widespread use is not justified.
