Infiltración del espacio epidural vía lumbar o caudal para el tratamiento de la lumbalgia aguda o subaguda : Evaluación de la eficacia a corto y medio plazo

Abstract

RESUMEN : Objetivos. Evaluar la eficacia de las infiltraciones epidurales sobre el dolor agudo/subagudo de raquis lumbar con ciatalgia, en una cohorte de pacientes y a corto plazo en el Hospital Universitario Marqués de Valdecilla. Metodología. Sobre una muestra de 100 pacientes derivados a la Unidad del dolor por presentar lumbociatalgia. Se registraron variables de edad, sexo, sociolaborales, la intensidad del dolor y la eficacia de las infiltraciones a las 2 semanas, 30 dias y 3 meses. Resultados. La edad media de los pacientes fue de 57 años. Previamente a las infiltraciones el 50% presentaba dolor severo (EVA >7). Las infiltraciones epidurales redujeron de forma significativa tanto el EVA medio como el porcentaje de pacientes con dolor severo y en cualquiera de los tiempos estudiados. Conclusiones. Las infiltraciones epidurales son eficaces en reducir significativamente el dolor lumbar agudo/subagudo a corto plazo. Queda por definir sus efectos a largo plazo.

ABSTRACT : Purpose. To evaluate the efficacy of infiltration of the epidural space on acute/subacute pain of lumbar spine with sciatica in a cohort of patients and in short term at Marqués de Valdecilla University Hospital. Methods. Applied on a sample of 100 patients referred to the Pain Unit due to low back pain. Variables of age, sex, socio-occupational status, intensity of pain and effectiveness of the epidural infiltrations were recorded at 2 weeks, 30 days, and 3 months. Results. The mean age of the patients was 57 years. Prior to the infiltrations, 50% had severe pain (VAS >7). The infiltration of the epidural space significantly reduced both: the mean VAS and the percentage of patients with severe pain at any of the times studied. Conclusions. The infiltration of the epidural space is effective in significantly reducing acute/subacute low back pain in the short term. Its long-term effects remain to be defined.
