Estudio identificativo del anciano de alto riesgo en un Servicio de Urgencias Hospitalario

Abstract

RESUMEN: Los servicios de urgencias hospitalarios (SUH) han aumentado la presión asistencial en los últimos años, así cómo la edad de los pacientes que atienden. En los últimos años, las altas hospitalarias de pacientes mayores de 75 años han pasado de 41,67/10.000hab en 2009 a 71,57/10.000 hab en 2019. Además, en el año 2050, la población mayor de 65 años alcanzará el 31,4% frente el 18,7% actual. El envejecimiento está asociado a una pérdida de la capacidad intrínseca del individuo, la capacidad de adaptarse al entorno, interactuar con el, siendo el fallo el desencadenante de la discapacidad. Y con ello la aparición de la fragilidad, síndrome de disminución de las reservas de la persona que aumenta la vulnerabilidad a resultados adversos. La mortalidad es el evento adverso más frecuente, siendo entre 1,3 y 7,3 veces más elevado el riesgo de mortalidad. La fragilidad tiene una prevalencia de un 20% aproximadamente. La escala Identification Seniors at Risk (ISAR) es una escala diseñada en Canadá para la detección del anciano de riesgo, con una sensibilidad del 85%. OBJETIVO GENERAL: Analizar la capacidad de detección de la escala ISAR del anciano de alto riesgo en los pacientes de más de 65 años atendidos en un Servicio de Urgencias Hospitalario. MATERIAL Y MÉTODOS: Estudio observacional prospectivo que se llevará a cabo mediante la valoración de pacientes mayores de 65 años en el Servicio de Urgencias del Hospital Universitario Marqués de Valdecilla (HUMV) por parte de enfermeras. Se valorará la dependencia, mortalidad, hospitalización, reingreso en hospital, revisita al SUH y el estado cognitivo de los sujetos a los 30, 90 y 180 días desde el alta hospitalaria.

OBJETIVE: To analyze the detection capacity of the ISAR scale of the high-risk elderly in patients over 65 years treated in a Hospital Emergency Service. MATERIALS AND METHODS: A prospective observational study that will be carried out by evaluating patients over 65 years of age in the Emergency Service of the Marqués de Valdecilla University Hospital (HUMV) by nurses. Dependence, mortality, hospitalization, hospital readmission, ED visit and cognitive status of the subjects at 30, 90 and 180 days from hospital discharge will be assessed.
