Proceso de acompañamiento en final de vida por covid-19: características e impacto para pacientes, familiares y profesionales sanitarios

Abstract

RESUMEN: El 31 de enero de 2020 se tomó conciencia de la gravedad de la enfermedad Covid-19. Una pandemia cuyo impacto fue visible a nivel socio-económico, psicológico y sanitario. Este trabajo, cuya confección se ha acotado a información recabada hasta el 1 de abril de 2021, pretende mostrar una realidad vivida desde la aparición del virus SARS-CoV-2. Por una parte, la importancia de unos cuidados paliativos de calidad, puesto que se encuentran centrados en la asistencia a nivel biopsicosocial de los pacientes Covid-19 positivos graves en situación de últimas horas/días de vida. No sólo en lo referente al manejo sintomatológico, sino encaminado a paliar uno de los efectos colaterales de esta enfermedad, que además, ha evidenciado aún más la necesidad de la humanización de la asistencia: la soledad de la muerte en tiempos de pandemia. Por otro lado, un colapso del sistema sanitario muy prolongado en el tiempo que ha fomentado la sobrecarga física y emocional del personal en primera línea, repercutiendo a su vez sobre la calidad de los cuidados del paciente. El objetivo de la presente monografía es el de analizar las características y el impacto en el proceso de acompañamiento en final de vida de pacientes Covid-19 positivos graves, sus familiares y las enfermeras, tras la declaración de la pandemia. Es importante resaltar que gran parte de la estrategia necesaria para poder sortear las dificultades que entraña esta infección, precisa de la participación e implicación de las profesionales que proporcionan cuidados a pie de cama.

ABSTRACT: On 31 January 2020, awareness of the seriousness of the Covid-19 disease was raised. A pandemic whose impact was visible at the socio-economic, psychological and health levels. This thesis, which has been compiled on the basis of information collected up to 1 April 2021, aims to show a reality experienced since the appearance of the SARS-CoV-2 virus. On the one hand, the importance of quality palliative care, since it is focused on the biopsychosocial care of severe Covid-19 positive patients in the last hours/days of life. Not only in terms of symptomatological management, but also with a view to alleviating one of the collateral effects of this disease, which has also further highlighted the need for the humanisation of care: the loneliness of death in times of pandemic. On the other hand, the collapse of the healthcare system over a very long period of time that has led to the physical and emotional overload of frontline staff, which in turn has had a direct impact on the quality of care of the patient. The aim of this monograph is to analyse the characteristics and impact on the end-of-life care process of severe Covid-19 positive patients, their families and nurses, since the declaration of the pandemic. It is important to emphasise that a large part of the strategy needed to overcome the difficulties involved in this infection, requires the participation and involvement of the professionals who provide care at the bedside, the nurses.
