Eficacia del gel de plasma rico en plaquetas autólogo en úlceras de pie diabético neuropático

Abstract

RESUMEN: 1.1 – Introducción Hoy en día las heridas crónicas y en concreto las heridas de pie diabético neuropático, suponen una importante repercusión psicológica y social para el paciente que las presenta. Estas lesiones son una de las complicaciones de la diabetes y según algunos estudios publicados, de un 15% a un 25% de usuarios que padecen esta patología sufrirán alguna lesión a lo largo de su enfermedad. Dada la importancia que conlleva, es necesario avanzar en nuevas técnicas para el tratamiento de estas lesiones con tórpida evolución. El PRPA en gel podría ser una herramienta barata, útil y fácil de usar para mejorar la cicatrización. Este producto se caracteriza por tener una concentración de plaquetas superior a la cantidad habitual en sangre, las cuales son activadas con cloruro de calcio, de esta forma liberan factores de crecimiento y citoquinas que son necesarios para la cicatrización. 1.2 – Objetivos Determinar la eficacia del PRPA en forma de gel para mejorar la cicatrización de las heridas de pie diabético neuropático. 1.3 – Metodología Se realizará un estudio experimental, en concreto un ensayo clínico aleatorizado y prospectivo, donde habrá dos grupos de investigación. Uno de los grupos será tratado con cura en ambiente húmedo y otro grupo será tratado con PRPA gel. La muestra estará formada por 218 sujetos, por asignación aleatoria simple, que cumplan los requisitos de inclusión y exclusión. El evaluador encargado de la comparación entre los grupos determinará si existen diferencias entre los grupos. 1.4 – Plan de trabajo La investigación será llevada a cabo durante el período comprendido entre el 1 de septiembre de 2019 y el 1 de septiembre de 2020, ambos inclusive. Los usuarios que pertenezcan a la misma, tras ser aleatorizados serán incluidos al grupo que pertenezcan. En el grupo 1, las curas serán tratadas con el método de cura en ambiente húmedo, y el grupo 2 será tratado con el PRPA gel. Las mediciones de las heridas se realizarán con el método Kundin (Superficie (cm2)= largo x ancho x 0,785). Para registrar los cuidados y la evolución de las lesiones, serán usados unos cuestionarios de evolución y serán cumplimentados por las enfermeras colaboradoras del CHUIMI. Los datos obtenidos se clasificarán en un programa estadístico llamado Epidat 4,2 mensualmente. Tras obtener todos los resultados del tratamiento de todas las lesiones incluidas en la investigación, se procederá a la comparación de dichos resultados y se definirá si existe diferencia entre ambos tratamientos.

ABSTRACT: 3.1 – Introduction Nowadays chronic wounds and specifically neuropathic diabetic foot wounds imply an important psychological and social impact for the patient who presents them. These wounds are one of the most commons of diabetes’ complications and according to some published studies, from 15% to 25% of users who suffer from this pathology will endure some injury throughout their illness. Given the importance that it entails, it is necessary to advance in new techniques for the treatment of these lesions, with a torpid evolution. PRPA gel, could be a cheap, useful and easy to use tool to improve healing. This product is characterized by having a platelet concentration higher than the amount in blood, which are activated with calcium chloride, thus releasing growth factors and cytokines that are necessary for healing. 3.2 – Objective To determine the effectiveness of PRPA, in gel form, to improve the healing of neurotic diabetic foot wounds. 3.3 – Methodology An experimental study is going to be carried out, a randomized and prospective clinical trial specifically, with the participation of two research groups. One of the groups is going to be treated with the wet cure, and another group is going to be treated with PRPA gel. The sample will consist of 218 subjects, by simple random assignments, who meet the inclusion and exclusion requirements. The comparison between the groups will determine if there are differences between the groups. 3.4 – Plan of work The investigation is going to be launched during the period between 1st of September of 2019 and 1st of September 2020 inclusive. Pertaining users, after being randomized and included in one of the two groups, are going to be treated according to the group they belong to. In group 1, cures are going to be attended with wet methodology, and group 2 is going to be treated with the PRPA gel. Wound measurements are going to be made with Kundin method (Surface (cm 2) = length x width x 0,785). Care and evolution of the lesions recording and evolution questionnaires are going to be used and completed by collaboration of nurses, members of traumatology and vascular services of CHUIMI. Obtained data are going to be classified in a statistical program called Epidat 4.2 monthly. After obtaining all the results of the treatment of all the injuries included in the investigation, we are going to be able to proceed with results comparison, and it will be defined if there is a difference between both treatments.
