Autoinmunidad asociada a la inmunoterapia anti-tumoral

Abstract

RESUMEN: La terapia antitumoral mediante inhibidores de puntos de control inmunitario (immune checkpoints o ICIs) está siendo una de las vías de investigación con una perspectiva más prometedora de los últimos años, llegando a su punto álgido con la consecución del Nobel de Medicina en 2018. No obstante, por su mecanismo de acción, estas estrategias terapéuticas tienen el riesgo de provocar reacciones autoinmunes. Tales efectos se han denominado por ello "immune-related adverse effects" (irAEs). En este trabajo, se analizan las interacciones entre el sistema inmunitario y el tumor, el papel de los ICIs y de sus inhibidores, profundizando en la epidemiología, las posibles formas clínicas de presentación, y las medidas terapéuticas propuestas para contrarrestar los irAEs.

ABSTRACT: Immune-checkpoint inhibitors (ICIs) as anti-tumoral therapy has been one of the most developed and promising pathways of investigation in the recent years, reaching its peak with the 2018 Nobel prize in Medicine award. Nevertheless, the novel adverse effects of these therapies, named immune-related adverse effects (irAEs) due to its underlying mechanism, have unique characteristics and implications as a result of the nature of these biological treatments. In this work, we discuss the interaction between immune system and cancer, the role of immune checkpoints and their inhibitors and, more importantly, the epidemiology, clinical presentation and management of irAEs.
