Arteria peronea: estudio anatómico e implicaciones clínicas en el tallado de colgajos locales perforantes para reconstrucción de región maleolar lateral

Abstract

RESUMEN: Introducción. La región maleolar lateral presenta una vascularización a expensas, primordialmente, de ramas de la arteria peronea. Esta región puede permitir la posibilidad de tallado de colgajos locales. Por otra parte, las complicaciones quirúrgicas en esta región requieren un estudio anatómico exhaustivo. Objetivo. Estudio de la rama principal terminal de la arteria peronea en la región maleolar lateral, la arteria calcánea lateral. Implicaciones clínicas. Material y métodos. Estudio anatómico de la región maleolar lateral, enfatizando el estudio en la arteria calcánea lateral y sus relaciones con referencias anatómicas constantes. Análisis estadístico de los resultados obtenidos. Resultados. La presencia de la arteria peronea fue constante. La arteria calcánea lateral estuvo presente en todos los casos, ya fuese como arteria principal o en forma de arco anastomótico con las arterias dorsalis pedis o tibial posterior. Entre las diferentes medidas realizadas, destaca la mayor variabilidad de las mismas en las correspondientes a la transición arteria peronea‐arteria calcánea lateral, con las implicaciones terapéuticas que conlleva. Conclusión. La presencia constante de la arteria calcánea lateral en la región maleolar lateral presenta, sin embargo, variaciones anatómicas con implicaciones terapéuticas en el tallado de incisiones en dicha región, que deben ser más posteriores e inferiores. Debe respetarse la disección de espesor total en la disección de los tejidos blandos (plano subperióstico).

ABSTRACT: Introduction. The lateral maleolar region presents a vascularization at the expense,primarly, of branches of the peroneal artery. This region may allow the possibility of carving local flaps. On the other hand, surgical complications in this region require an exhaustive anatomicals study. Purpose. Study of the main terminal brach of peroneal artery in the lateral malleolar region, the lateral calcaneal artery. Clinical implications. Material and methods. Anatomical study of the lateral malleolar region, emphasizing the study of the lateral calcaneal artery and its relations with constant anatomical references. Stadistic analysis of the obtainted results. Results. The appearance of the peroneal artery was constant. The lateral calcaneal artery was present in all cases, either as the main artery or in the shape of anastomotic arch with the dorsalis pedis or posterior tibial arteries. Among the different measures carried out, the greater variability stands out in those corresponding to the transition between the peroneal artery and the lateral calcaneal artery, with the therapeutic implications that this entails. Conclusion. The constant presence of the lateral calcaneal artery in the lateral malleolar region presents, however, anatomical variations with therapeutic implications in the carving of incision in that region, wich must be posterior and inferior. The full thickness dissection in the dissection of the soft tissues (subperiostical plane) must be respected.
