Impacto de la musicoterapia y la realidad virtual sobre el grado de ansiedad de los pacientes sometidos a cirugía traumatológica con anestesia raquídea: ensayo clínico

Abstract

La ansiedad es una de las complicaciones más comunes que sufren los pacientes durante el proceso asistencial sanitario. En situaciones delicadas como una cirugía, los niveles de ansiedad son mayores y, por tanto, más difíciles de tratar. Se ha estudiado la aplicación de terapias no farmacológicas en diferentes ámbitos sanitarios. En el caso de la ansiedad perioperatoria, destacan por encima del resto la Musicoterapia y la Terapia de Exposición de Realidad Virtual, siendo esta última un tipo de intervención reciente que está obteniendo buenos resultados en varios estudios. Objetivos: comparar el impacto que tiene la Musicoterapia y la Terapia de Exposición de Realidad Virtual sobre la ansiedad intraoperatoria de los pacientes quirúrgicos de traumatología que son anestesiados a nivel raquídeo, además de analizar el grado de reducción de la ansiedad en cada caso y estudiar el grado de satisfacción tras la cirugía. Material y Métodos: se creará un grupo experimental para la Musicoterapia, otro grupo experimental para la Terapia de Exposición de Realidad Virtual y un grupo control para la atención estándar del área quirúrgica del Hospital Universitario Marqués de Valdecilla. Para la Musicoterapia se usarán unos auriculares inalámbricos con los que se reproducirá una lista de canciones durante la intervención quirúrgica. En el caso de la Terapia de Exposición de Realidad Virtual, se utilizará el dispositivo Oculus GO y se reproducirá una lista de vídeos a través del mismo. En todos los grupos se utilizará el cuestionario STAI para analizar la ansiedad y la escala EVA para el dolor, ambas antes y después de la cirugía. La frecuencia cardíaca y la presión arterial se medirán antes, durante y después de la intervención. El grado de satisfacción se evaluará con una encuesta cuyas variables se definen mediante el modelo SERCAL, siendo esta adaptada a un servicio quirúrgico. El análisis estadístico se realizará con el paquete estadístico IBM SPSS Statistics 22.

Anxiety is one of the most common complications suffered by patients during the healthcare process. In delicate situations such as surgery, anxiety levels are higher and, therefore, more difficult to treat. The application of non-pharmacological therapies in different healthcare areas has been studied. In the case of perioperative anxiety, Music therapy and Virtual Reality Exposure Therapy stand out above the rest, the latter being a type of recent intervention that is obtaining good results in several studies. Objectives: to compare the impact of Music Therapy and Virtual Reality Exposure Therapy on the intraoperative anxiety of surgical trauma patients who are anesthetized at the spinal level, in addition to analyzing the degree of anxiety reduction in each case and studying the degree of satisfaction after surgery. Material and Methods: an experimental group will be created for Music Therapy, another experimental group for Virtual Reality Exhibition Therapy and a control group for the standard care of the surgical area of Hospital Universitario Marqués de Valdecilla. Music therapy will use wireless headphones with which a list of songs will be played during the surgical intervention. In the case of Virtual Reality Exposure Therapy, the Oculus GO device will be used and a list of videos will be played through it. In all groups, the STAI questionnaire will be used to analyze anxiety and the EVA scale for pain, both before and after surgery. Heart rate and blood pressure will be measured before, during and after the intervention. The degree of satisfaction will be evaluated with a survey whose variables are defined by the SERCAL model, being this adapted to a surgical service. The statistical analysis will be carried out with the statistical package IBM SPSS Statistics 22.
