La atención del paciente pluripatológico: importancia del papel de la enfermera de atención primaria

Abstract

RESUMEN: La sociedad actual se encuentra evolucionando hacia un perfil poblacional muy diferente al de cualquier época pasada. La esperanza de vida se encuentra en pleno auge, creciendo exponencialmente desde finales del siglo XX. Por otro lado las patologías crónicas están imponiéndose frente a las agudas aumentando las tasas de prevalencia e incidencia en estos grupos de riesgo. Esta situación arrastra consigo la necesidad de responder al Reto de la Cronicidad elaborando diferentes estrategias con las que poder abordarla. Entre estas cabe destacar la Ruta del Paciente Pluripatológico desarrollada en Cantabria con el fin de clasificar a cada paciente en diferentes grupos aportando los cuidados necesarios y asegurándose de una correcta continuidad asistencial en los casos de de inestabilidad. Finalmente cabe destaca el papel fundamental de los profesionales enfermería de Atención Primaria mediante un abordaje holístico del paciente, además de evolucionar hacia nuevas figuras como es la enfermera gestora de casos o a la enfermera de continuidad asistencial o enlace enfermeras de enlace.

ABSTRACT: The current society is evolving towards a population profile very different from that of any past era. Life expectancy is booming, growing exponentially since the late twentieth century. On the other hand, chronic diseases are prevailing against acute diseases, increasing the prevalence and incidence rates in these risk groups. This situation brings with it the need to respond to the Challenge of Chronicity by developing different strategies with which to address it. Among these, it is worth highlighting the Path of the Pluripathic Patient developed in Cantabria in order to classify each patient in different groups providing the necessary care and ensuring a correct continuity of care in cases of instability. Finally, it is worth highlighting the fundamental role of the nursing in Primary Care through holistic approach of patients as well as evolving towards new figures such as nurse case manager, liaison nurses or nursingo continuity nurse .
