Aplicaciones reconstructivas de los colgajos derivados del eje circunflejo femoral lateral

Abstract

La versatilidad ofrecida por el eje circunflejo femoral lateral permite la obtención de distintos tipos de colgajos según la demanda de la zona a reconstruir, gracias a la posibilidad de variaciones de tamaño, composición, traslocación, lo que hace posible la obtención de un colgajo más adaptado a la lesión y, por tanto, con un mejor resultado estético, convirtiéndose, así, en una opción de primera línea para el cirujano plástico dentro de las técnicas reconstructivas. En nuestra serie se incluyen las distintas modalidades de colgajos basados en perforantes de este eje vascular, empleados en las intervenciones reconstructivas realizadas por el servicio de Cirugía Plástica del Complejo Asistencial Universitario de León, quedando reflejada su utilidad en la reconstrucción de varias regiones corporales, de donde se extrapola la posibilidad de su empleo para reconstrucción en una gran variedad de defectos de partes blandas. Este trabajo trata de revisar los aspectos más importantes de los colgajos derivados del eje circunflejo femoral lateral, su anatomía y la del eje vascular que presentan en común, las técnicas de disección quirúrgica, su diseño y elevación, las distintas modificaciones y aplicaciones que permiten, así como un análisis de sus ventajas y desventajas.

The versatility offered by the lateral femoral circumflex axis allows obtaining different types of flaps according to the demand of the area to be reconstructed, thanks to the possibility of variations in size, composition, translocation, which makes it possible to obtain a more adapted flap to the lesion and, therefore, with a better aesthetic result, thus becoming a first line option for the plastic surgeon within the reconstructive techniques. Our series includes the different modalities of flaps based on perforators of this vascular axis, used in the reconstructive interventions performed by the Plastic Surgery service of the University Care Complex of León, and it is reflected on the usefulness in the reconstruction of several body regions, from which is extrapolated the possibility of its use for reconstruction in a great variety of soft tissue defects. This work tries to review the most important aspects of the flaps derived from the lateral femoral circumflex axis, its anatomy and the one of the vascular axis that they present in common, the techniques of surgical dissection, its design and elevation, the different modifications and applications that allow it, as well as an analysis of its advantages and disadvantages.
