Comparación de la evolución postoperatoria de la resección hepática laparoscópica y abierta en la cirugía del hepatocarcinoma

Abstract

INTRODUCCIÓN: La literatura publicada sobre las ventajas de la cirugía laparoscópica sobre la abierta en el tratamiento del hepatocarcinoma es escasa hasta la actualidad. OBJETIVOS: Comparar la evolución postoperatoria en la cirugía hepática laparoscópica y abierta, en términos de complicaciones (intraoperatorias y postoperatorias) y estancia hospitalaria. PACIENTES Y MÉTODOS: Desde Enero de 2014 hasta Marzo de 2018, 35 pacientes fueron sometidos en el Hospital Universitario Marqués de Valdecilla (HUMV) a cirugía electiva (20 por laparoscopia y 15 por laparotomía). En esta cohorte retrospectiva se compararon aspectos demográficos, complicaciones y estancia hospitalaria. RESULTADOS: La tasa de complicaciones postoperatorias (p=0,019) y de gravedad (p=0,044) fue significativamente inferior en el grupo laparoscópico, así como ausencia de diferencias en la tasa de positividad en los márgenes (p=0,661) y aunque no de forma estadísticamente significativa, una tendencia a una menor estancia hospitalaria en el grupo intervenido por abordaje laparoscópico frente al tratado mediante cirugía abierta.CONCLUSIONES: La tasa de complicaciones postoperatorias y su gravedad han resultado inferiores en el grupo laparoscópico. No se ha demostrado mayor positividad en los márgenes en el grupo laparoscópico. Existe una tendencia a una menor estancia hospitalaria en el grupo laparoscópico.

INTRODUCTION: There is not much published literature to date about the advantages of laparoscopic surgery over open surgery in the treatment of hepatocellular carcinoma. OBJETIVES: To compare the postoperative outcomes between laparoscopy and open hepatic surgery in terms of complications (both intraoperative and postoperative) and hospital stay. PATIENTS AND METHODS: From January 2014 to March 2018, a total of 35 patients were submitted to elective surgery at the Marqués de Valdecilla University Hospital (HUMV) (20 by laparoscopy and 15 by laparotomy). In this retrospective cohort, demographic aspects, complications and hospital stay were compared. RESULTS: There is a significant lower rate of postoperative complications (p = 0.019) and severity (p = 0.044) in the laparoscopic group, as well as a lack of differences in the margin positivity rate (p = 0.661). Although a non-significant difference, a tendency to a shorter hospital stay in the group operated by laparoscopic approach compared to those treated by open surgery was found. CONCLUSIONS: The rate of postoperative complications and its severity have been statistically lower in the laparoscopic group. No greater positivity has been demonstrated in the margins in the laparoscopic group. There is a tendency to a shorter hospital stay in the laparoscopic group.
